A health assessment is a longer, more comprehensive appointment with our Practice Nurse and your GP.
Importantly, the purpose of this health assessment is to help identify any risk factors that may require further health management. In addition to assessing a person’s health status, a health assessment is used to identify a broad range of factors that influence a person’s physical, psychological and social functioning.

45-49 year old Health Assessment
A health assessment at this stage of life can assist patients to make the necessary lifestyle changes to prevent or delay the onset of chronic disease
75 years and older
The purpose of this health assessment is to help identify any risk factors exhibited by patients over 75 years of age that may require further health management. This health assessment is used to identify factors that influence a person's physical, psychological and social functioning and is conducted on an annual basis
Aboriginal and Torres Strait Islanders
The aim is to help ensure that Aboriginal and Torres Strait Islander people receive health care matched to their needs, by encouraging early detection, diagnosis and intervention for conditions that cause morbidity and early mortality. The health assessment for Aboriginal and Torres Strait Islander people covers the full age spectrum
To book your health assessment call us on 61220150 – please inform our Reception Staff that you'd like to have a Health Assessment when booking your appointment, so that sufficient time with the Practice Nurse and GP can be arranged.
There are no out-of-pocket costs for any of these assessments.

Pre-employment functional assessments
A pre-employment functional assessment is a physical examination conducted by our accredited
Exercise Physiologist, Rojina.
Functional examinations are used to determine the candidate’s ability to meet the physical requirements of their role
During the functional assessments you are assessed for; range of motion, grip strength, positional tolerances, lifting and carrying capacity and cardiovascular fitness
Diving Medicals should ideally be conducted by a Doctor who has had the approved training to medically examine recreational and commercial (occupational) divers.
Dr Graham Stevens is a member of SPUMS, the South Pacific Underwater Medicine Society and provides both recreational and commercial diving medicals. Dr Stevens holds both the Diploma in Diving Medicine from SPUMS and the Advanced diploma in Hyperbaric and Diving Medicine from the Australian And New Zealand College of Anaesthatists.
Dr Stevens works part time as a consultant/staff specialist in the department of Hyperbaric and Diving Medicine at the RHH.
Our Diving Medical Certificates are in accordance with the Australian Standard AS4005.1 Compliance and ANZ 2299.1-3 (2015).
Diving Medicals take approximately one hour to complete. Having an appointment is no guarantee of fitness to dive and further tests or examinations may be required.
(GP Management Plan and Team Care Arrangement)
CARE PLANS
Care plans, also known as GP Management Plans are available for our regular patients with ongoing medical conditions lasting more than 6 months.
A Care Plan is a written plan of management developed by your GP and practice nurse consultation with you. It is a written set of information about what you need in managing your medical condition.
All Care Plans are bulk billed by your GP. There will be no charge for these services.

Once a Care Plan has been created you may be eligible to see any allied health professional for 5 visits per year subsidised under Medicare under a Team Care Arrangement.

Care Plan Reviews
Once a plan is in place, it should be regularly reviewed by your GP. This is an important part of the plan cycle, where you with your GP and practice nurse check that your goals are been met and agree on any changes that you may require. A Care Plan Review is usually under taken every 6 months. All Care Plan Reviews are subsidised under Medicare. There will be no charge for these services.
Your Health Hub participates in the Close the Gap program run by Medicare for our indigenous patients.
We encourage our patients who identify as Aboriginal or Torres Strait Islander to let us know so that we can best help you manage your health, as part of the Close the Gap program your doctor will also offer you a preventative health check to help identify and treat any health problems you may have.
We will ask all of our patients: “Are you of Aboriginal or Torres Strait Islander origin?”
To register for the Closing The Gap scheme at Your Health Hub, we will need you to sign a form stating that you want Your Health Hub to be your usual care provider, and to look after your chronic disease and/or chronic disease risk factors.
A new form will need to be completed every year, and we can do this from the 1st November to cover for the following year
Patients registered at Your Health Hub (YHH) under the Closing The Gap scheme of any age will receive discounted consultations.
Patients registered at Your Health Hub (YHH) under the Closing The Gap scheme who have a current Concession Card will be bulk-billed.

At Your Health Hub Closing The Gap patients will be offered a FREE annual ATSI Health Check with a Practice Nurse and their GP, every 9 to 12 months.
Each ATSI Health Check is tailored to the individual, taking into consideration age, gender, family history, risk factors, lifestyle, and existing medical conditions. An ATSI Health Check takes about one hour.

Registering at Your Health Hub under the CTG scheme also improves access to PBS medications for those living with, or at risk of chronic disease. Presciptions printed for Closing The Gap patients will display a CTG PBS Co-Payment code. The PBS Co-Payment Measure reduces the cost of PBS medications at the Pharmacy. Closing the Gap prescriptions attract a lower co-payment for PBS medicines. Concession Card Holders will be dispensed their PBS medications for FREE at the Pharmacy.

There are various fees depending on which procedure you may need. Our charges cover the cost of using the treatment room, the materials as well as nurse assistance to make sure your procedure goes as smoothly as possible.
